Notice to All Patients
To request copies of your medical records, please complete the Medical Records Request Form on our website or call our office at (979) 776-2828 during normal business hours.
Requesting Medical Records
In accordance with Texas Administrative Code Rule §76.2, patients may request that their medical records be disclosed either to themselves or to another person or entity.
Requests for records should be submitted in writing and include:
- The specific records or information being requested.
- The name of the person or organization that should receive the records.
- The patient’s signature or the signature of an individual legally authorized to act on the patient’s behalf.
Upon receipt of a valid request and any applicable fees, records will generally be provided within fifteen (15) business days unless disclosure is otherwise prohibited by law.
Filing a Complaint
If you have concerns regarding the care provided by this office, you may file a complaint with the appropriate licensing or regulatory agency.
